Chronic Obstructive Pulmonary Disease (COPD) is a debilitating disease with increasing morbidity and mortality worldwide. The main goal of therapy is symptomatic relief and improvement of quality of life through different medical treatment. However, such therapies cannot reverse or significantly reduce hyperinflation leaving patients with poor quality of life. In a selected subgroup of patients with predominant emphysema and hyperinflation, bronchial valve therapy represents a further treatment option, recently approved by the FDA. Recently, trials have also focused on bronchoscopic treatment of chronic bronchitis. The purpose is to provide a comprehensive summary of the currently available clinical trials, discuss common complications and their management as well as evaluation of proper patient selection in clinical practice.

Upon completion of this activity, participants will be able to:

• Review recent medical advances in treatment of COPD.
• Review evidence based clinical trials for valve therapy in severe emphysema.
• Able to recognize the predictors of success for valve therapy in severe emphysema.
• Describe complications related to valve therapy and how to manage them.
• Recognize patients who candidate for valve therapy are as compared to surgical lung volume reduction in daily clinical practice.
• Identify patients who might be candidates for chronic bronchitis trials. 
• Review the role of surgery in treatment of serve emphysema.
